I do not take insurance and do not bill insurance companies directly. However, I can provide itemized receipts for patients to submit to insurance companies. It is the patient's responsibility to determine if they are eligible for any reimbursement.

Payment is expected at the time of service unless other arrangements have been made, and patients are required to maintain an active card on file. I accept major credit and debit cards, including Visa, Mastercard, American Express, and Discover. I also accept most HSA and FSA debit cards.

I require 24 business hours advanced notice for cancellations or the patient will be responsible for the full fee. For example, Monday appointments must be canceled by the prior Friday.
